German text in the Gesangbuch:

1 Ich weiß, an wen ich gläube:

mein Jesus ist das Glaubens Grund,

bei dessen Wort ich bleibe,

und das bekennet Herz und Mund.

Vernunft darf hier nichts sagen,

sie sei auch noch so klug;

wer Fleisch und Blut will fragen,

der fällt in Selbstbetrug.

Ich folg in Glaubenslehren

der heilgen Schrift allein;

was diese mich läßt hören,

muß unbeweglich sein.

2 Herr, stärke mir den Glauben;

denn Satan trachtet Nacht und Tag,

wie er dies Kleinod rauben

und um mein Heil mich bringen mag.

Wenn deine Hand mich führet,

so werd ich sicher gehn;

wenn mich dein Geist regieret,

wirds selig um mich stehn.

Ach, segne mein Vertrauen

und bleib mit mir vereint;

so laß ich mir nicht grauen

und fürchte keinen Feind.

3 Laß mich im Glauben leben;

soll auch Verfolgung, Angst und Pein

mich auf der Welt umgeben,

so laß mich treu im Glauben sein.

Im Glauben laß mich sterben,

wenn sich mein Lauf beschließt,

und mich das Leben erben,

das mir verheißen ist.

Nimm mich in deine Hände

bei Leb- und Sterbenszeit,

so ist des Glaubens Erde

der Seelen Seligkeit.

My prose translation:

1 I know in Whom I would believe;

My Jesus is the foundation of faith

With Whose word I remain

And Whom heart and mouth confess.

Reason may say nothing here,

However clever it may be;

He who wants to ask flesh and blood

Falls into self-deception.

I follow the teachings of faith

Of the Holy Scripture alone;

What this lets me hear

Must be immovable.

2 Lord, strengthen my faith;

For Satan strives night and day

How he may steal this gem

And murder my salvation.

When Your hand leads me

I will securely go;

When Your Spirit governs me,

It will stand around me blessed.

Oh, bless my trust

And remain united with me

Thus I let myself not dread

And fear no enemy.

3 Let me live in faith;

Even should persecution, fear, and torment

Surround me in the world,

Let me be true in faith.

Let me die in faith

When my course ends,

And [let] me inherit the life

That is promised to me.

Take me into Your hands

At the time of life and the time of death;

Thus is the end of faith

The salvation of the soul.

As far as I can tell, this hymn isn't in The Lutheran Hymnal, Lutheran Worship, or The Lutheran Service Book.  According to the Gesangbuch, the text is sung to the tune "Nun lob, mein Seel, den H[erren]."  Here's the arrangement from TLH:
