German text in the Gesangbuch:

1 O Lämmlein Gottes, Jesu Christ,

der du mein Trost und Leben bist,

ich armer Sünder komm zu dir

und bring viel Missethat mit mir.

2 Ach Gott, ich hab gesündigt sehr

und mir gemacht ein Bürden schwer;

doch bitt ich wollst mir gnädig sein

und nehmen weg all Schuld und Pein.

3 Wie Sanct Johanns, der Täufer, mich

nur alles legen heißt auf dich,

denn du seist da vom Himmelszelt,

zu helfen mir und aller Welt.

4 Forthin will ich gern bessern mich,

deim Wort gehorchen williglich;

drum, o Herr, bleib allzeit bei mir

und nimm mich endlich gar zu dir.

My prose translation:

1 O Lamb of God, Jesus Christ,

You Who are my Comfort and Life,

I, a poor sinner, come to You

And bring many misdeeds with me.

2 O God, I have sinned much

And made a heavy burden for myself;

Yet I ask [that You] will be merciful to me

And take away all guilt and torment.

3 Like Saint John, the baptist, calls me

Simply to lay everything on You,

For You would be there from the tent of Heaven

To help me and all the world.

4 Henceforth, I want gladly to improve myself,

To obey Your word willingly;

Therefore, O Lord, remain with me always

And take me finally completely to You.

I'm not sure I have the right sense of the third verse.

As far as I can tell, this hymn isn't in The Lutheran Hymnal, Lutheran Worship, or The Lutheran Service Book.  According to the Gesangbuch, the text is sung to the tune "Herr Jesu Christ, meins."  Here's the TLH arrangement (transposed from Bb major to C major):
